Leave Application for Creative Arts Day Camp

According to the Nomination Guideline of the Scheme, all Arts Ambassadors are required to attend the Creative Arts Day Camp. Scholarship will be given to the students as a reward for the completion of all core activities.
If you were late, took early leave or absent from the Creative Arts Day Camp which was held in April, you are required to submit a Leave Application Letter with teacher's signature and school chop to our office via email or fax (2970 3809) by 11 May 2015 (Mon) as a record. Please note that scholarship will not be given to the Ambassador if no valid Leave Application Letter is received by 11 May 2015.
